Forms Explained

Below is a list of LMU's most commonly used academic forms, with instructions for using them. If you do not have access to a printer, come pick up a printed copy in CFA's Academic Advisement Center (St. Robert's Hall, Suite 102). Each of these forms involves slightly different steps for completion, but all end with dropping off at the CFA Academic Affairs Office (St. Robert's Hall, Suite 101) for the associate dean's signature.

  • Degree Audit Adjustment Form

    Degree Audit Adjustment Form

    When To Use:

    You'll use this form to substitute core, flag, or major and minor requirement courses on your Degree Audit Report.

    How To Use:

    • All Degree Audit Adjustment Forms must be signed by your dean.
    • If the substituted course is a major requirement, you must also get the form signed by your department chair. If the adjustment pertains to a flag or core requirement, you do not need a chair signature.
    • Drop off completed form at the CFA Academic Affairs Office (St. Robert's Hall, Suite 101) for associate dean's signature. Associate dean's office staff will email or call only if there are follow up questions.
  • Change of Program Petition

    Change of Program Petition Form

    When to Use:

    You'll use this form to switch majors or add a major or minor.

    How to Use:

    • Fill it out and seek signature from the chair or program director for your added program; if you are dropping a major or minor, also seek signature from the chair or director of the program you are leaving.
    • Drop off completed form at the CFA Academic Affairs Office (St. Robert's Hall, Suite 101) for associate dean's signature. Associate dean's office staff will email or call only if there are follow up questions.
    • NOTE: if the program you are adding or dropping is in another college, you will need to seek deans' signatures from the corresponding dean's office. For example, if you are removing a communication studies minor and adding a business administration (CBA) minor, you would need to seek a signature from the CFA dean's office to remove philosophy and a signature from the College of Business Administration dean's office to add business administration.
  • General Petition

    When to Use:

    You'll use the above forms to make adjustments to your schedule. Use the late add petition to add or withdraw from a course after registration ends; use the registration adjustment form to apply for a unit overload to allow you to take more than 18 units in a semester; use the exception to university policy form to change course status to credit/no-credit or audit and other special circumstances with courses.

    How to Use:

    • The approval signatures on this form vary by issue, but are named clearly on the form.
